چکیده

This report reviews key principles of quality (as it relates to the overall quality of the health care system and individual approaches to quality improvement); reviews evidence of the existence and root causes of racial and ethnic health disparities and recommendations to address them; and discusses strategies by which the quality and cultural competence movements could be linked. In particular, it focuses on the Institute of Medicine’s six principles for designing a highquality health care system to identify areas where aspects of cultural competence would be central to achieving high quality. It then presents a framework outlining both hypothetical and proven strategies for delivering high-quality, culturally competent care. Support for this research was provided by The Commonwealth Fund. Pay-for-performance programs to reduce racial/ethnic disparities: limitations of a one-size-fits-all approach.

Pay-for-performance (P4P) programs are broadly defined as performance-based payment arrangements that are designed to promote improvement in health care quality while reducing costs. Often absent in the equation is the issue of disparities. Racial and ethnic disparities in health care persist, and some believe that P4P programs have the potential to exacerbate such inequities in the quality of ...

Beyond rhetoric: what we need to know to eliminate disparities.

Racial and ethnic disparities in health care are well documented and can be discussed in 3 broad categories: health system factors, patient-level factors, and patient/provider interaction. Clinicians and others working in health care and related fields are knowledgeable about disparities in health, but the general US population is not. Racial/ethnic disparities are most striking in life expecta...
